Translation notes:
- This statement runs only when `includeNonPrimaryTaxIds` is `true`. The follow-on provider load is SQL-204.
- `TaxId` is the entity Id (`ProviderTaxIdMap.cs:13`), so the ROWNUM limit is defensive only.
