Steps

1. Build a command against pk80001_LEGALNET.insertLetterRequest with CommandType.StoredProcedure (LegalManagerBase.vb, ~line 396).
2. Bind letterTypeCode, aLegalCaseNum, holdDate, createUser, aClaimId, and requestNum (InputOutput).

3. Execute; the returned requestNum is used by callers to associate the request.
